Ischemic cardiomyopathy: Simultaneous epicardial and endocardial mapping and ablation

Abstract

A 65-year old male diagnosed with ischemic ventricular tachycardia (VT) underwent ultra-high density electro-anatomic and VT ablation. Simultaneous epicardial-endocardial mapping of VT revealed a circuit with complete endocardial reentry along border zone of basal inferior scar with single loop reentry and incomplete reentry epicardially correlating with basal inferior scar. These observations confirm the functional involvement of the epicardium in the mechanism of ischemic VT, suggesting the role of combined endo-epicardial approach as a first-line strategy in some selected cases.
